Submit your application online by filling out the online form below and pressing the submit button once.  Please fill out as many fields as possible.  We especially need your name and a means to contact you. 

Then, we will send a verification letter to the mailing address you provide.  Bring this letter along with a picture i.d. to the library to pick up your new library card.  You've been carded!

If you are under 18, you must have your parent or guardian accompany you to the library with his or her picture i.d. and your verification letter in order to pick up your new library card.
